HER2 testing in breast cancer: Opportunities and challenges
Human epidermal growth factor receptor 2 (HER2) is overexpressed in 15-25% of breast cancers, usually as a result of HER2 gene amplification. Positive HER2 status is considered to be an adverse prognostic factor. Recognition of the role of HER2 in breast cancer growth has led to the development of anti-HER2 directed therapy, with the humanized monoclonal antibody trastuzumab (Herceptin (R)) having been approved for the therapy of HER2-positive metastatic breast cancer. Clinical studies have further suggested that HER2 status can provide important information regarding success or failure of certain hormonal therapies or chemotherapies. As a result of these developments, there has been increasing demand to perform HER2 testing on current and archived breast cancer specimens. This article reviews the molecular background of HER2 function, activation and inhibition as well as current opinions concerning its role in chemosensitivity and interaction with estrogen receptor biology. The different tissue-based assays used to detect HER2 amplification and overexpression are discussed with respect to their advantages and disadvantages, when to test (at initial diagnosis or pre-treatment), where to test (locally or centralized) and the need for quality assurance to ensure accurate and valid testing results
Spatially Resolved Magnetic Field Structure in the Disk of a T Tauri Star
Magnetic fields in accretion disks play a dominant role during the star
formation process but have hitherto been observationally poorly constrained.
Field strengths have been inferred on T Tauri stars themselves and possibly in
the innermost part of the accretion disk, but the strength and morphology of
the field in the bulk of the disk have not been observed. Unresolved
measurements of polarized emission (arising from elongated dust grains aligned
perpendicular to the field) imply average fields aligned with the disks.
Theoretically, the fields are expected to be largely toroidal, poloidal, or a
mixture of the two, which imply different mechanisms for transporting angular
momentum in the disks of actively accreting young stars such as HL Tau. Here we
report resolved measurements of the polarized 1.25 mm continuum emission from
HL Tau's disk. The magnetic field on a scale of 80 AU is coincident with the
major axis (~210 AU diameter) of the disk. From this we conclude that the
magnetic field inside the disk at this scale cannot be dominated by a vertical
component, though a purely toroidal field does not fit the data well either.
The unexpected morphology suggests that the magnetic field's role for the
accretion of a T Tauri star is more complex than the current theoretical

On the Gold Standard for Security of Universal Steganography
While symmetric-key steganography is quite well understood both in the
information-theoretic and in the computational setting, many fundamental
questions about its public-key counterpart resist persistent attempts to solve
them. The computational model for public-key steganography was proposed by von
Ahn and Hopper in EUROCRYPT 2004. At TCC 2005, Backes and Cachin gave the first
universal public-key stegosystem - i.e. one that works on all channels -
achieving security against replayable chosen-covertext attacks (SS-RCCA) and
asked whether security against non-replayable chosen-covertext attacks (SS-CCA)
is achievable. Later, Hopper (ICALP 2005) provided such a stegosystem for every
efficiently sampleable channel, but did not achieve universality. He posed the
question whether universality and SS-CCA-security can be achieved
simultaneously. No progress on this question has been achieved since more than
a decade. In our work we solve Hopper's problem in a somehow complete manner:
As our main positive result we design an SS-CCA-secure stegosystem that works
for every memoryless channel. On the other hand, we prove that this result is
the best possible in the context of universal steganography. We provide a
family of 0-memoryless channels - where the already sent documents have only
marginal influence on the current distribution - and prove that no
SS-CCA-secure steganography for this family exists in the standard

SMART: Unique splitting-while-merging framework for gene clustering

The clustering performance degrades significantly unless parameters are properly set, and yet, it is difficult to set these parameters a priori. To address this issue, in this paper, we propose a unique splitting-while-merging clustering framework, named “splitting merging awareness tactics” (SMART), which does not require any a priori knowledge of either the number of clusters or even the possible range of this number. Unlike existing self-splitting algorithms, which over-cluster the dataset to a large number of clusters and then merge some similar clusters, our framework has the ability to split and merge clusters automatically during the process and produces the the most reliable clustering results, by intrinsically integrating many clustering techniques and tasks. The SMART framework is implemented with two distinct clustering paradigms in two algorithms: competitive learning and finite mixture model. Nevertheless, within the proposed SMART framework, many other algorithms can be derived for different clustering paradigms. The minimum message length algorithm is integrated into the framework as the clustering selection criterion. The usefulness of the SMART framework and its algorithms is tested in demonstration datasets and simulated gene expression datasets. Moreover, two real microarray gene expression datasets are studied using this approach. Based on the performance of many metrics, all numerical results show that SMART is superior to compared existing self-splitting algorithms and traditional algorithms. Three main properties of the proposed SMART framework are summarized as: (1) needing no parameters dependent on the respective dataset or a priori knowledge about the datasets, (2) extendible to many different applications, (3) offering superior performance compared with counterpart algorithms.National Institute for Health Researc
Psychometric analysis of the scale for the predisposition to the occurrence of adverse events in nursing care provided in ICUS
OBJECTIVE: to present the result of the validity and reliability studies concerning the Scale for the Predisposition to the Occurrence of Adverse Events (EPEA).
METHOD: construct validity was based on Principal Components Analysis.
RESULTS: reliability verified through Cronbach's alpha indicated good reliability (structure α=0.80; process α=0.92).
CONCLUSION: based on its psychometric indicators, the EPEA can be considered a valid measure to assess the attitudes of nurses in relation to factors that potentially lead to the occurrence of adverse events in ICUs
Paradigm of tunable clustering using binarization of consensus partition matrices (Bi-CoPaM) for gene discovery

Conventional binary and fuzzy clustering do not embrace the biological reality that some genes may be irrelevant for a problem and not be assigned to a cluster, while other genes may participate in several biological functions and should simultaneously belong to multiple clusters. Also, these algorithms cannot generate tight clusters that focus on their cores or wide clusters that overlap and contain all possibly relevant genes. In this paper, a new clustering paradigm is proposed. In this paradigm, all three eventualities of a gene being exclusively assigned to a single cluster, being assigned to multiple clusters, and being not assigned to any cluster are possible. These possibilities are realised through the primary novelty of the introduction of tunable binarization techniques. Results from multiple clustering experiments are aggregated to generate one fuzzy consensus partition matrix (CoPaM), which is then binarized to obtain the final binary partitions. This is referred to as Binarization of Consensus Partition Matrices (Bi-CoPaM). The method has been tested with a set of synthetic datasets and a set of five real yeast cell-cycle datasets. Scapular deformity in obstetric brachial plexus palsy: a new finding
While most obstetric brachial plexus palsy patients recover arm and hand function, the residual nerve weakness leads to muscle imbalances about the shoulder which may cause bony deformities. In this paper we describe abnormalities in the developing scapula and the glenohumeral joint. We introduce a classification for the deformity which we term Scapular Hypoplasia, Elevation and Rotation. Multiple anatomic parameters were measured in bilateral CT images and three-dimensional CT reconstruction of the shoulder girdle of 30 obstetric brachial plexus palsy patients (age range 10 months–10.6 years). The affected scapulae were found to be hypoplastic by an average of 14% while the ratio of the height to the width of the body of scapula (excluding acromion) were not significantly changed, the acromion was significantly elongated by an average of 19%. These parameters as well as subluxation of the humeral head (average 14%) and downward rotation in the scapular plane were found to correlate with the area of scapula visible over the clavicle. This finding provides a classification tool for diagnosis and objective evaluation of the bony deformity and its severity in obstetric brachial plexus palsy patients
